Home Modifications
for Aged Care Clients
Positive Step provides mobile occupational therapy services across Perth to support older adults in modifying their homes for improved safety, accessibility and independence.
If you or someone you care about is finding it harder to move safely and confidently around the home, the right modifications can make everyday tasks easier and reduce the risk of injury. At Positive Step, we take the time to understand what matters most to you, then recommend practical changes that support how you live day to day.
Our focus is to create a home environment that supports your safety, independence and quality of life, so you can continue living comfortably in your own home.
Home modifications under aged care funding
Home modifications may be funded under the Assistive Technology and Home Modifications (AT-HM) Scheme, part of the Support at Home program.
This funding supports changes to the home environment that improve accessibility, safety and function. Depending on your needs, this may include minor adjustments or more complex modifications to support mobility and daily living.
Positive Step works with you to assess your eligibility and provide clear recommendations aligned with funding requirements.
Types of modifications we may recommend
Home modifications are tailored to your individual needs, home layout and level of function. This may include:
- Installation of grab rails and handrails
- Ramps and access modifications
- Bathroom modifications, including level-access showers
- Recommendations for stair lifts or major access solutions
- Redesigns to improve accessibility and movement within the home
All recommendations are made in line with Australian Standards to ensure they are safe, compliant and practical.
How we assess and plan modifications
At Positive Step, we take a structured and practical approach to ensure any modifications are appropriate and effective.
We assess how you currently move and function within your home, identify any risks or barriers, and determine what changes will have the greatest impact on your safety and independence. Recommendations are tailored to your environment, routine and future needs.
